Health Blaenavon Survey Participation

Healthy Blaenavon can support you to live a healthy life. So far, the responses to their previous surveys have told them that:

  • Some people in Blaenavon would like to be healthier, to lose weight and be more active
  • Loneliness and isolation is a challenge and not just for the older population
  • People would like to know more about opportunities to get outside, including local walks and cycle routes

Over the next few weeks they’ll be promoting stories, information and support on each of these things and they would really like to hear from others across the community.

Patient Access Survey 2020/21

Be a participant and play an active role.

We would appreciate if you could spare 5 minutes to participate in a survey if you have had an appointment with any GP, nurse or pharmacist here at Blaenavon Surgery.

All answers are anonymous.

As a practice we are keen to review our appointment system. The findings from the survey will be reflected upon and will be displayed in the practice, on our Facebook and the practice website.

Additional support for patients


One step referral into all homelessness and housing-related support.  You may need help finding or sustain your tenancy due to mental health, domestic violence, support for independent living or financial concerns.  You’ll be asked some questions about your circumstances and directed to the most appropriate support.
