What is Social Prescribing?
Social Prescribers help you to access services providing social, emotional and practical support for issues that are affecting your health and well-being. These issues may be anything that is bothering you including housing and financial concerns, relationships, training and employment, healthy lifestyle support, loneliness and isolation. Appointments last up to 1 hour where you have the opportunity to talk through the issues that you are facing, explore what is important to you and find ways to access support.
We have appointments available on Thursday afternoons with our Social Prescriber, Jade. You may have seen the large poster in our waiting area. Please contact us if you wish to avail yourself of this valuable service.
Please note the survey linked below has now concluded. Thank you if you participated.
If you have attended a social prescribing appointment recently with Blaenavon Medical Practice, it would help greatly if you could participate in a survey to give feedback on your social prescriber experience. N.B. this links to a survey that is being run by Cardiff Metropolitan University.
Blaenavon Medical Practice will be closed on the 25th & 26th December 2018, and also 1st January 2019.
